Recall of a transfer is far more likely in the first 24 hours than at any point afterwards. Call the number on the back of your card — not any number you were given by the person who contacted you. In the UK you can dial 159 to reach most major banks' fraud lines directly. Come back and finish this afterwards.

Pick the closest match. If two apply, choose the one that cost you money.

Did you lose money?

There is a report worth filing either way. This only changes who else needs to hear about it.

What did they get?

Tick everything you gave them, entered on their site, or that they took.

Who were they?

Anything you have. Blank is fine — partial identifiers still help investigators join cases together.
